Campus Farmer's Market Vendor Application Form - Office of Sustainability

Please fill out the following form to submit your application for a vendor stall at the U of S Campus Farmer's Market.

Note: Vendors serving food will also have to fill out an additional Health Region Approval Form for the Health Inspector found HERE. Please fill out this form and e-mail a copy to campus.market@usask.ca.

By submitting this application form, vendors agree to abide by the following statements:

I have read and understand the Campus Farmer's Market Information Package
I understand that I am required to complete and submit a Health Region Approval form and submit it to the Office of Sustainability if I am selling food items.
I give permission to the University of Saskatchewan and the Univeristy of Saskatchewan Students' Union to use photos taken during the Campus Farmer's Market for promotional uses.
I give permission for my business information (business name and webpage) to be shared on the Campus Farmer's Market webpage.
